Cleaning the top of the carpet makes a urine spot look better for a week. The smell comes back because the urine went through the backing and into the pad.

Dried urine is often invisible. Before we start, we go over the rooms for the spots you know about and the ones you do not, and mark each one so nothing gets missed.
Treated as deep as it went
As urine dries it leaves salts behind that hold the smell, which is why it comes back on humid days. Each spot gets an odor treatment applied as deep as the urine went, then extraction to pull it back out, then a rinse.
When the pad has to go
If a spot has been soaked over and over, the pad under it holds more than any treatment can reach. The fix then is to pull the carpet back, replace that piece of pad, seal the subfloor if needed, and stretch the carpet back in. We do that too. See carpet repair.
